Documents the Wyoming Department of Transportation (WYDOT), Driver License Records asks for

Filling in the form is half of it. Wyoming also wants supporting documents at the counter:

  • Certified marriage certificate (or court order/divorce decree showing name change)
  • Current Wyoming driver's license or ID card (to surrender)
  • Proof that name has already been updated with the Social Security Administration
  • Payment for standard renewal fee ($35 for driver's license)
  • Proof of Wyoming residency if applying for REAL ID-compliant credential

REAL ID note: Must update name with Social Security Administration BEFORE applying. SSA record must verify the new name and date of birth before WYDOT will issue a revised license/ID. Applicant is re-photographed and must surrender the old card in person.

Good to know for Wyoming: Order online, in person, or by mail. Certified marriage certificate costs $25 (plus ~2.4%/$1 min convenience fee for online credit card orders). Requestor must be a party to the marriage or legal representative, and must provide a photocopy of a valid driver's license, state ID, or passport. Where to send each form

Two of these go in the mail, and they do not go to the same place.

Form SS-5. Wyoming is one of the states where the SSA supports a fully online marriage name change, so you may not need to mail anything at all. Start at my Social Security. If you would rather use paper, take or mail the SS-5 to a local office: find your nearest one.

Passport forms. This is where Wyoming matters, because routine DS-82 service is split by state. Wyoming is routed to the Pennsylvania processing center.

Expedited service uses different addresses again and costs extra, and the instructions ask you to write "Expedite" on the outside of the envelope. Use a trackable mailing service: that envelope holds your passport and your certified certificate.
