• Current Ohio driver's license or state ID card to surrender/update
• Proof of Ohio residency (e.g., utility bill or bank statement) if updating to a REAL ID-compliant credential
• Certified copy of any divorce decree or annulment if a prior marriage name change is also being unwound

REAL ID note: A name change must be done in person at an Ohio BMV/deputy registrar location; it cannot be completed online. Ohio recommends updating your name with the Social Security Administration first, since BMV verifies your SSN record electronically before issuing the new license/ID.
