Change Your Name on Your Social Security Card in Illinois

In Illinois, as everywhere in the U.S., your Social Security record is the anchor every other agency checks against. If you visit the Illinois Secretary of State (ILSOS) - Driver Services before the SSA has processed your change, the names won't match and you'll be turned away. That's why Form SS-5 is always step one.

The SS-5 is completely free. Never pay a third party just to file it. You can complete it by mail or at a local Social Security office.

To update your Social Security card you'll generally need:

• A certified copy of your marriage certificate (order yours from the Office of the County Clerk (in the Illinois county where the marriage occurred) - the Illinois Dept. of Public Health Division of Vital Records does NOT hold marriage records and cannot issue certified copies)
• Proof of identity (your current Illinois driver's license or ID)
• A completed Form SS-5

Good to know for Illinois: Illinois marriage certificates are recorded and certified only at the county clerk level, not by the state. IDPH's Division of Vital Records can only provide a $5 verification-of-facts letter (names, DOB, event date/place) for marriages from 1962 onward, by mail/fax/in-person - not an official certified certificate.
