  • Completed Form D1 (Application for Alaska Driver License, Permit or ID Card)

  • Proof of Social Security number/legal presence documents if required for REAL ID

Alaska does not accept the license name change online. A visit to Alaska Division of Motor Vehicles (DMV) is required.

REAL ID in Alaska

Name changes cannot be done online; must appear in person at an Alaska DMV office within 30 days of the legal name change (AS 28.05.071, per the DMV's own page), complete Form D1, and may need to pass a vision test. A REAL ID-compliant credential requires additional proof of identity/residency documents.
