How U.S. Bank says to do it
- In person: For checking and savings, its page says you can change your name at any branch, and links to make an appointment. It also lists a phone number but does not say the change can be finished by phone.
Documents it mentions
- An updated government-issued ID or passport
- Legal name change documents it lists as accepted: a Social Security card, a marriage certificate, a divorce decree, a court document with an official seal, stamp or authorized signature, or naturalization papers
- It says it does not accept a marriage license unless it also works as a certificate or includes the marriage date, witness signatures or a notary seal
