How Delta Air Lines says to do it
- Online: For your SkyMiles account, its help page points to an Identity Verification Form (you log in to reach it) where you can submit your supporting documents online.
- By mail: Its SkyMiles program rules also allow the documents to be mailed to the address listed there.

- A copy of supporting legal documentation, for example a court order, marriage certificate or divorce certificate
- Identity verification, which it says is required for any name change request
- Your signature and SkyMiles number on any written correspondence
