How Bank of America says to do it
- In person: For a personal account, its page says to schedule an appointment in a financial center, where an associate gives you forms to complete and helps with a new debit or credit card. If you are not near one, it gives a customer service number for information on how to make the change, and it does not say the change can be finished by phone. Its business and Private Bank pages differ.

- A valid government-issued photo ID
- One of: a marriage certificate, a divorce decree indicating the name change, a court order of name change, or an adoption certificate
- For an account with several owners, it says all account owners need to be present at the appointment with valid government-issued IDs
