Change your name with AT&T

How AT&T says to do it
- By mail: A legal name change is requested by letter, sent by U.S. mail to the address for your service. For AT&T wireless it says you can fax the documents instead.
Documents it mentions
- Its requirements: you are the account holder, the account has been active for more than 30 days, the change is for a legally qualifying reason, and the new name matches your new legal name on a valid photo ID
- A letter with the date you want the change, your account number (or wireless numbers), a current phone number, a return address, your current name and your full new legal name, and your signature
- Documents showing your previous legal name (matching your current bill) and your new legal name, for example a marriage certificate, a divorce decree or a driver's license
